Our Services
Community First Services & Supports (CFSS)
CFSS is a flexible, person-centered program that provides assistance with:
Activities of Daily Living (ADLs): Eating, grooming, dressing, bathing, mobility, toileting, transferring, and positioning.
Health-Related Tasks: Tasks delegated by licensed professionals, such as medication reminders and health monitoring.
Instrumental Activities of Daily Living (IADLs): Meal planning, shopping for essentials, household chores, managing finances, and community participation.
Behavior Support: Redirection, intervention, observation, and monitoring
245D Waiver Services
Designed to assist individuals with disabilities or aging-related needs, 245D Waiver Services helps clients gain independence and integrate into their communities. These services include a wide range of support tailored to each individual’s care plan.
Respite Care offers temporary relief for caregivers, allowing them to take time for themselves while ensuring their loved ones receive professional care.
In-Home Respite: Delivered in the comfort of the individual’s home.
Out-of-Home Respite: Provided in a safe and supportive facility.
Individualized Home Support (IHS)
Flexible, person-centered services delivered in three formats:
With Family Training: Training family members to provide effective care for their loved ones.
With Training: Helping individuals develop life skills to achieve greater independence.
Without Training: Direct assistance with daily tasks and activities to maintain independence.
Adult Companion Services
Providing companionship and emotional support while assisting with light activities to help clients feel engaged and supported in their daily lives.
Homemaking Services
Our Homemaking Services aim to maintain a clean and organized living environment for ourclients. Services include:
– Light housekeeping and laundry
– Meal preparation and planning
– Shopping for essentials
Individual Community Living Supports (ICLS)
ICLS focuses on building skills and providing support to help individuals live independently in their communities. This includes:
– Skill-building activities
– Health management support
– Emotional and social engagement
Night Supervision
Overnight care is designed to ensure safety and provide support for individuals who need assistance during nighttime hours.
